It's important to get to know your little readers' habits so you can best support them. This includes not only what books they like to read but what books they aren't connecting with. It is also important to help your readers learn how to self-reflect on their own reading habits.
This simple and quick-to-complete form is something you can ask your 3rd-5th graders to fill out whenever they "abandon" a book. Then use the completed forms as a springboard for a meaningful one-on-one conference and keep on file where you may even start to notice reader's patterns. The form also includes an example form filled out by a student.
