This 15 page form is for groups to use in what I call "Book Clubs". I use Book Clubs with groups of 5 students, where they sit and read a chapter together and then discuss and fill out the response form. This form is designed for a book with 15 or fewer chapters; my students usually read Magic Tree House, Junie B. Jones, A to Z Mysteries, Horrible Harry, and many other age appropriate chapter books. It is very open ended, so it could be used with virtually any chapter book at many different grade levels. After reading a chapter, one student summarizes, another asks a relevant question, another recalls an important event from the chapter, another connects to something in their personal life, and the last student predicts what will happen next. These same activities are completed after each chapter, but they rotate so that each student has a chance to do each one at least once. Bonus idea: When my students finish their book and their form is completed, I have them complete a creative book report of their choosing, like a diorama or designing a book cover. My students love this activity!
