Use these Nonfiction and Fiction Book Forms to monitor independent reading from your students. It is similar to a book report but less painful and to the point. The student will respond to essential questions that will allow you to quickly assess if the student has grasped the important aspects of the book.
I hang a chart up in the room with the students’ names on it. I set a goal of 25 books for them to read in one school year. Once they finish reading the book, they fill out a Reading Form. When finished, they submit it to me for review. Once approved, I tell them to put stickers on the book chart. Some books are work more stickers than others based on content, complexity, and amount of pages. Their goal is reach 25 stickers.
