This is a book report form I use in my classroom. I created it to give kids a choice for doing a book report instead of everyone having to do the same thing. I noticed that my students reports were much better when given the option of how to present their report. Everyone fills out the same packet, but they can choose a presentation method - such as artistic (create a poster), scrappy (make a scrapbook), dramatic (act out as the character), constructive (create a scene), or choose their own (most pick power point or windows movie maker for their own).
