This activity would work best with shorter books (picture books, easy readers, guided reading selections, etc.) and would great with primary students. However, I also used it with my 4th graders as an introduction to our unit on writing book reviews. They will work their way up to more involved book reviews on a book at their reading level.
The first sheet includes characters, setting, beginning, middle and end of the story and their favorite part of the story. Students also get to provide a rating for the book (out of 5 stars) and create their own cover art for the book.
The second sheet includes title, author, a quick fill-in-the-blank review, a favorite part, and a rating of the book. They can also create their own cover art for the book on the back page.
You can copy front to back as an in-class assignment or for homework. You could also copy separately and display side-by-side on construction paper for display.
