This is a 2 page Word document with approximately 13 questions related to the reading - read a few paragraphs, answer the questions, read a few paragraphs, answer the questions. The KEY is included. The questions are directly related to the reading without having to search the entire document for the answers. No additional material is needed to complete this assignment. It's all in the document. No prior knowledge is required to answer the questions. There are also several full-color pictures and a bunch of interesting factoids to keep the student interested in the content. I usually use this assignment toward the middle of our respiratory unit or at the end as a refresher.
This assignment is used on the computer with a drop box such as Blackboard or School Space. It can be quickly and easily converted into a printable paper copy by simply removing the asterisks and adding lines for the students to write their answers instead of typing them. I use this format often so that we can save paper by using electronic copies of assignments.
The student answers the questions by placing the cursor between the red asterisks and begins typing the answer - that way they can easily see that they've answered the question and I can easily grade the assignment by reading what they've typed in red. The student's completed answer sheet will look much like the teacher's answer key when done correctly. I love technology!
A printed, paper copy can be easily created by removing the red asterisks and adding lines for the student to write their answers. Word documents are so easy to manipulate and tweak. Make it yours! However, even when a student is absent during the day we use this assignment, I simply give him the printed copy without any tweaking - red asterisks included - and there's usually enough room for the student to write their answers next to the asterisks.
