As a music teacher, I have a different class every 45 minutes. When I send a student out to a buddy room, I need a way to keep track of it, because I will forget by the end of the day who I sent, and why.
I created this page to help me with documentation. I print my pages on card stock, although you could just as easily print them on paper. When I send a student out, I cut the ticket on the dotted line, and send the left side with them.
I write the time their class is over on their ticket before they leave, so the Buddy teacher knows when they will be leaving, in case their class is leaving the room before then to go to recess, a Special, etc.
On the right side, I fill out the information. For the time slot, I write the time the student left my room, and time the class was over. That way in the future, I can track how long they were out of my room. On the back, I write why I sent them. I send them with a think sheet, but even though I tell them why I am sending them out of the room, their perception and my reason don't always match.
After the student returns, I staple the ticket to the think sheet. I keep these for my files. This is how I use it, but you should adapt it to meet your needs. I hope it's a help to you, no matter what your specialty!
