This is a daily Positive Behavior Support (PBS) daily Check-In Check-Out (CICO) form for Kinder through 3rd graders. I have used this form with students in my resource room to support monthly progress monitoring for IEP goals and with Response to Intervention (RTI) Tier II students. At the end of each thirty day period encourage students to fill out (or talk through if you are brave and working with the kinders) and use the discussion questions to support students in self-reflection. This intervention can help empower students to take ownership of their behavior and support the development of a meaningful student/teacher relationship.
