I have been using these for years in my classroom. I print them on colored paper for a monthly theme. Students get one when they are seen making positive choices. These can range from cleaning up, helping someone, taking risks, and participating.
Students write their names on the back and I have a plastic bin they put them in. At the end of the week, I pull a few names (I keep track) and they get a special reward.
It is amazing when you ask a question and only a few raise their hands and then you say, "for a Caught-Ya Card". Then you get at least 10 more hands up!
I like to use it in transitions, mystery walkers, and bucket filling.
