I'm an SLP traveler... so I have a new caseload ever year. I have to do chart reviews of every student at the beginning of every year. I don't have that familiarity with my students as a lot of therapists have. I created this sheet so that I could have a reference in my data notebook when I need quick information and don't want to pull out the working file and/or IEP on the computer. It's a time saver, and let's face it, we all need as much time as we can possibly get!! It's also a great way to get organized at the beginning of the year, and I know I'm always looking for an easy way to organize a lot of information in a small space. (Paper is a precious commodity where I work!)

SLP Tip: This form is very busy, and has a lot going on. I try to write each section in a different color just so it doesn't make you dizzy trying to read it! I also have my own personal laminator (one of the small $20 ones from walmart- AMAZING) and I like to laminate and/or put in protective sheets to keep them from getting damaged.
