This is a Positive Behavior Support (PBS) daily Check-In Check-Out (CICO) form for third through 8th graders. I have used this form with students in my resource room to support monthly progress monitoring for IEP goals and with Response to Intervention (RTI) Tier II students. At the end of each thirty day period encourage students to fill out the graph and use the discussion questions to support students in self-reflection. This intervention can help empower students to take ownership of their behavior and support the development of a meaningful student/teacher relationship. Enjoy!
