This checklist includes the basics in reading and math. Included are 2 teacher checklists and 24 sheets from which students can respond. Cards are provided from which to arrange numbers and alphabet letters in order. In some cases, manipulatives from the classroom will be needed to assess a skill, such as counting coins or telling time. Directions giving suggestions on how to use the checklist are provided.

The checklists focus on:
*identification of colors and color words
*identify and write letters of the alphabet
*identification of letter sounds (including blends, consonant digraphs and patterns, vowel digraphs and long vowel patterns, r-controlled vowels)
*identification of positions
*identify and write personal information
*spell color words
*identify and spell number words
*identify words for days and months
*identification of sight words
*identification of shapes
*counting with one on one correspondence
*identify and write numerals
*arrange and write numerals in order
*count orally
*skip count by twos, fives, tens
*identify and count coins
*telling time

Response sheets are provided for:
*identification of colors and color words

*identification of words for days and months
*identification of sight words
*identification of shapes
*identification of numerals
*arrangement of numerals using flashcards
*flash cards can also be used for skip counting
*identification of coins

Teachers are often called on to do an informal assessment of a student, especially resource teachers. This checklist can be used as a starting point, showing what a child has and has not mastered.

I hope this checklist is a simple resource you can use to provide a documentation of a child’s skills.
