Chlamydia is on the rise in most states of our nation, especially amongst sexually active teens. It is a silent disease (few signs or symptoms) that can lead to sterility if untreated. Students are not as uncomfortable as you may think about researching this information and actually have many side-bar discussions about chlamydia while completing the assignment. This is probably due to the gross-out factor of an STD. The answers to this 12 question assignment can be found on the CDC website which is embedded into the assignment.
This assignment is a Word document that is used on the computer with a drop box such as Blackboard, PowerSchool or School Space. It can be quickly and easily converted into a printable paper copy by simply removing the asterisks and adding lines for the students to write their answers instead of typing them. I use this format often so that we can save paper by using electronic copies of assignments.
The student answers the questions by placing the cursor between the red asterisks and begins typing the answer - that way they can easily see that they've answered the question and I can easily grade the assignment by reading what they've typed in red. A nice feature of assignments that are used on the computer along with a website is that the assignment can be 'pinned' to one side of the computer screen and the website pinned to the other so the student can see both the assignment and the website they're working from at the same time. I love technology!
However, if you prefer a paper copy, simply remove the red asterisks and add lines as a place for students to put their answers. The assignment was created directly from the CDC website, but this can be used as a webquest or a research assignment instead.
