I use this as a type of contract to assist students in recognizing between good and bad behaviors in my autism class. It could be used in any classroom. First students mark the time down when they made a good or bad choice. Then circle good or bad above the box and if they like they can draw a happy or sad face. Higher students could make a check mark for good choice or x mark for a bad choice. The lines are provided for a short description of the choice. I send this home at the end of the day so my parents can have a discussion with the students of their behavior.
