Do you use Class Dojo? It's a free site for behavior management. I recently became a Class Dojo Mentor for being one of the early starters on Class Dojo. This year, in an effort to give both positive and negative points more frequently without interrupting a lesson to do it, I started a new system. This form is a modified version of my other tracking sheet. Later in the year I simplified the behaviors I was tracking and combined it with our daily log so that everything would be in one place. This is front and back, with a spot for tracking the daily percentages and other important info at the top, in addition to the detailed spot to track tallies throughout each day. My students are given two opportunities to record their points during brain breaks throughout the day. One is mid-day and one is at the very end of the day. I love this sheet because it serves as another way to report to parents, gives my students a way to self-monitor, and gives a paper reporting system for those who don't follow Dojo using the app. It is made in Microsoft Word so you can customize it for the Dojo behaviors that you track.
