These "tickets" are used in a classroom or school-wide reward system.
Anytime a class is caught doing something that a teacher (or staff member, if the system is being implemented school-wide) would like to reinforce (standing in line quietly, great behavior during PE/music/art, following directions during lunch, etc.), the teacher/staff member provides the class with an ICU. A student from the class should complete the ICU by entering the action that earned the ICU and filling in the name section with their teacher's name or the name of the class if appropriate. ICUs can be collected in a common place (a jar, a box, on the teacher's desk, etc.) and then randomly drawn at the end of the day, the week, the quarter, and/or at random to earn a reward.
The staff line is intended for the teacher/staff member to sign their name as proof that it came from an appropriate member of staff. For convenience's sake, teachers may wish to sign every ICU on one full page and hold this as an original from which to make copies. This way, ICUs do not need to be signed individually or as they are given out - they are already signed as you hand them out!
For my school specifically, ALL staff members make sure to have ICUs on hand at all times - in a pocket, attached to a lanyard, anywhere! This way, ICUs can be given anytime including when passing someone in the hall or on the playground. For this to work effectively, it would be best to implement the program school-wide or grade-wide to allow as many members of staff the opportunity to reward students and classes.
