I have had lots of students with food allergies. As their teacher, I am not always informed right away and I have had to go to the nurse’s office to sort through her records for each student to see what their allergies are. This is why I ask about it on my beginning of the year parent form and I record it for myself to keep in my teacher binder on this Allergy Record Form.
This form is very helpful when well-meaning parents and students bring snacks to share with the class. Now when this happens, I can check against my list to ask what the ingredients are and check to make sure each student can participate in the snack.
I include an extra column for Other Health Concerns, just in case a student is lactose intolerant, gluten intolerant, diabetic, or has any other food related health concern I need to remind myself of when we have food in the classroom.
