This document is designed to enhance collaboration between occupational therapists and classroom teachers. It is especially useful when working to support students who are not demonstrating the carryover of learned skills into the classroom, and for those students who may be undergoing a screening process or are benefiting from consultation.
This form should be completed collaboratively with the school occupational therapist and the referring teacher. I hope that by breaking down what “neat” writing is, and by giving teachers a glimpse through “OT eyes,” this process can help everyone be successful!
This download includes a history form, initial and follow up data sheets, and a summary of strategies. I also included data sheets with blank categories for individual preferences in describing handwriting legibility.
