This Discipline Summary Report called a BRICk (Behavior Report for Individual Clarification) is designed for the middle and upper level students to specifically communicate the inappropriate behavior that been observed and the student's own explanation of the event. It is designed to allow the teacher to quickly note the offense and segregate the student for a brief period of time to allow class to continue while the student is being made aware of his or her inappropriate behavior.
The form takes only seconds for the teacher to note the facts observed and no more than 3 to 5 minutes for the student to record his or her portion, take a quiet time out, then rejoin the class. The form can then be used as documentation if needed. This form is EDITABLE! If you improve on it, please send me a copy!
