This set of 15 flower-themed classroom forms includes both color and grayscale (black and white) copies. Included: bathroom passes, classroom sign-out sheet, extra help sign-in sheet, grade tracker, hall passes, homework passes, online textbook passwords, open house sign-in sheet, passwords (general), problem of the day organizer, quiz evaluation, table of content for binder, textbook sign-out sheet, absent slips, and blank chart.
Description of Included Items:
-Boys, girls, and general passes are included.
-I recommend hanging them by your classroom door and asking students to place the pass on a table when out of the room. This will alert everyone that someone is out of the room without ending up with a germy pass.
Classroom Sign-Out Sheet
-Keep by the door so that students can sign out and in throughout the day.
Extra Help Sign-In Sheet
-When students seek extra help, ask them to sign in so you can easily document your time and the reason.
-Appropriate for a points-based grading system.
-Students fill in the assignment, the score they receive, and a running total of all of the points they have earned over the total number of available points.
-The average is calculated by dividing the points they’ve earned by the total available points and multiplying by 100.
-NOTE: This does not work when assignments carry different percentage weights.
-Appropriate to use when students head to library, office, other classroom, etc.
-Pass out on a regular basis or as prizes.
Online Textbook Passwords
-At the beginning of the year, have students write down their online textbook information in several places, including this sheet for you to hold onto in your filing cabinet.
Open House Sign-In Sheet
-When parents visit for Open House or Parent-Teacher Conferences, ask them to sign in so you can remember who was present.
-Do your students have passwords for other sites that they need to keep track of? This sheet will help you keep them all organized.
-You can use this as a class list for a given website and keep it in your filing cabinet.
-Or you could give a copy to each student if they have lots of passwords to keep track of.
Problem of the Day
-Organizes two weeks worth of Problem of the Days, Bellwork, Do Nows, etc.
-This is a great reflective process that I have always used after a quiz. Students fill in:
-Name of the Assessment
-Strength – This is something they did well.
-Challenge – This is something they struggled with.
-For each question where they lost points, students fill in the problem number, topic, number of points lost, then they check whether it was a simple mistake or they didn’t understand.
-Corrections or reflection are completed on the back.
Table of Contents for Binder
-Great way to keep students organized!
Textbook Sign-Out Sheet
-Use at the beginning and end of the school year to keep track of textbooks.
-Use a different page for each class and/or book.
“While You Were Out…” Absent Slips
-Great resource for helping students stay organized if they have been out!
-Have a designated student fill in the information and attach it to any handouts from that day. Allocate space in the classroom for the absent work to go so that students can easily access it when they return. I hang absent work on a “While You Were Out…” bulletin board.
-For miscellaneous use as needed.
