It's reassuring to have a ready-to-use behavior modification plan on hand, especially when problematic behaviors occur. When students display inappropriate behaviors, giving them an opportunity to think about their actions and change them is a worthy tactic over doling out discipline or punishment. Rather than writing students up, allow the students themselves to write about the difficulties they are involved with and develop potential solutions on their own. While there are no guarantees that that you’ll experience positive results, young people, who are on the verge of making many important decisions on their own, need opportunities to attempt to rectify their own missteps and make a change of course. This action plan provides a place for that problem/solution process to occur.
The students I've used this with have told me that they appreciate an opportunity to make changes before I make contact with administration or a parent. And, while you may never need it, the responses of your student provide documentation of your attempts to rectify the behavioral issues if further action or intervention is warranted.
1. One “front” page with the most common behavioral concerns in checklist form. (8 items and an “other” space for other behaviors you'd like to include.)
2. Two different reverse sides for student responses—one with a space for teacher feedback, the other without. In the three student response sections, students are asked to analyze their behaviors, thinking of the effect of those behaviors on others and themselves; the benefits of curtailing their inappropriate behaviors; how to avoid reoccurrences of the behaviors; and how they can make amends for what they have done.
