Hold students accountable for their classroom behavior and improve your classroom management. This worksheet is designed to be filled out by a student that has received warnings for their actions, and still has not corrected their behavior. It helps students acknowledge that their disruptions take away from their peers, their teacher, and themselves. In this 2-page document (which can be printed front to back), the student will have to outline their behavior, how it affected others, and how it can be avoided in the future. It requires students to create a short plan of action to help them get back on track to learning without disruptions.
Suggestion: I usually give the student two warnings before I assign a Behavior Reflection to be completed during lunch detention, after school, etc.
