Use this form to help guide those students who are struggling with classroom behavior. Write a quick note about problem behaviors in each section or draw a smiley face or place a sticker in each section if the student did not have any problems during that time frame. Reward the student at the end of the day or week for the amount of smiley faces / stickers. This form also allows you to have documentation of the student's behavior in each class. The form can be folded in half (so no other students see the inside) and taped down to the student's desk. Teachers can easily walk by and jot down a quick note or draw a smiley face.
