This document contains a behavior management log and an editable letter addressed to parent and student. The Responsibility Log manages behaviors in the classroom setting. The teacher and student monitor behaviors and can circle the appropriate rule that needs improvement as needed. This system is quick and easy to administer during a busy class and it reinforces the students' responsibilities within the system. I have used this system with small variations made to the included forms and letters in both the elementary classroom and the middle school levels. Typical consequences for the Log are for a detention and phone call home following the three circles within a month. Followed by a referral if the student accumulates three more circles. Students must sign the log daily if they acquire logged data. This variation works well with older students and quickly promotes the students' role in the system. Examples of use in an elementary classroom had students staple the log to the take-home folder and a signature was acquired nightly from a parent.
