Three Self Evaluation Forms to be used with the whole class or with individual students as needed, to assist in regulating behavioural issues in the classroom.

Self Evaluation in the following areas:
• Treating others with Respect
• Listening
• Waiting my turn to talk (raising my hand)
• Sitting at my desk
• Doing my best work

Form 1 – to be used at the end of the day – student self-reflects on their overall behaviour and checks the boxes that apply most to them. Includes an area to reflect on how they can improve for tomorrow.

Form 2 – to be used in the morning and in the afternoon – student self-reflects on their behaviour at 2 points during the day and checks the boxes that apply most to them. Includes an area to reflect on how they can improve for tomorrow.

Form 3 - to be used at the end of the day – student self-reflects on their overall behaviour – all areas of the rubric are written out. Student circles the boxes that apply most to them. Includes an area to reflect on how they can improve for tomorrow.
