I have used point sheets in my classroom for students who struggle with controlling themselves, to stay on task or complete work. They can be used to try to reinforce positive behavior as well. The point sheet goes in a folder so other students don’t read the private “contract” between you and your student. The student who the point sheet belongs to is responsible for taking the folder with them between classes and to specials each day. I request that a parent signs the point sheet each night and return it to school the next day. This is valuable documentation you will want to file. Remember to be flexible and change the goals as needed so that your student finds success. I have included two versions. The first is appropriate for 2nd-4th grade. The second version is appropriate for younger students.
The files are Word documents so you can edit and customize for your particular student.
