Do you need a simple, easy way to obtain academic and behavioral information for students in your school? This evaluation tool will provide you with the information you need to support and write clear data-based Individual Education Programs (IEPs) present levels of performances (PLOPs), data- supported special needs Functional Behavioral Assessments (FBAs), and Behavioral Intervention Programs (BIPs). It is also a helpful tool to assist in gathering observational data for Child Study and RTI plans.
This Teacher Rating Report in an easy-to-use cued response Likert scale format. This rating scale has been tested over a five year period at the upper elementary levels and middle school levels, grades 6 through 8. When compared with an open-ended questionnaire, teachers prefer this format because it provides for both quantitative as well as qualitative data and is fast and easy to complete. My return rate from teachers using this format is drastically increased over my previous open-ended teacher report format. Teachers are more positive about completing them and they do so more quickly, because they are written in a cued response format. Teachers need not complete the entire report, only the sections you highlight.
Results allow you to compare behaviors of concern across classrooms as well as calculate an average rating for each targeted behavior. Two open ended questions are included so that teachers can add a personal opinion about the student’s strengths as well as areas of concern. A sample present level of performance and functional behavioral assessment data table is included to demonstrate ways the data can be used in its final form.
