This is a document to help first time therapists or counselors help to stay organized. This could also be for someone who wants to become more organized. I used this form when I was working at an elementary school and middle school and it helped aid in keeping all my ducks in a row. I knew when I first started out in Social Work it was important to keep all my clients/children, phone contacts, and miscellaneous information available to me at any given time. Having this list also made it easy for when I would meet for supervision with my manager I could easily use this list to go through each client.
