This 8 1/2 x 11 communication sheet goes home on a weekly basis to communicate students' behavior throughout the week. At the end of each day, students are responsible for coloring in their color from the clip chart. Any time students clip down, they must write the reason underneath "clip down" and at the end of the day the teacher marks his/her initials after reviewing. At the bottom, there is a parent signature line so that you know parents reviewed the behavior chart over the weekend.
**I use these at the end of each day when students fill in their clip chart color. Then, at the end of each week, these communication sheets go into students' Friday folders. The parent/guardian has to sign it and the student must return it on Monday. This not only helps hold students accountable, but it also holds me accountable to communicate all behaviors with parents.
