This form is to help general education teachers and special education teachers keep track of their co-teaching efforts. It is quick and simple to use.
Pre-service teachers and supervising professors/mentors can use this form to record observations of co-teaching.
In the first column, write in the date, circle the day of the week, write the time period of the co-teaching, circle the quarter of the school year, and circle the class period.
In the second column, write about the students and how they are grouped or how you and your co-teacher decided to group the students. Consider things such as special education, talented and gifted, grouping by ability, grouping by preference, and grouping by random.
In the third column, describe the instruction and activity of the class. Be sure to include what both teachers and the students were doing and the responses.
In the final column circle the method of co-teaching used. If more information is needed on each type of co-teaching, I encourage you to read anything by Marilyn Friend. Be sure to also note the effectiveness of the method chosen and determine whether or not to use that method again for similar activities.
