This portfolio has everything you need to get your students organized and accountable. I use this with my 4th and 5th grade students as an ongoing teaching and assessment tool. At the beginning of the year we set binders up and discuss how it will be used throughout the year to store notes, handouts, assessments and objectives/I can statements. This portfolio includes:
-Cover page with name
-Common Core standard recording sheet (includes a place for students to document the standards essential questions)
-Evidence of Learning. Students will write a list of I Can statements they are responsible for learning throughout the standard. On this page they will also record pre and post assessments for each standard.
-Domain Specific Vocabulary for each standard. Use this to document and define critical words needed for understand each standard
-Personal Word Wall with teacher notes and initials for check-in. The possibilities are endless with a personal word wall. I have students put one word wall behind their literature tab and one behind their informational tab. We use these in small group and independent reading.
-Year long documentation of affixed words (1 sheet- just print the number needed for the entire year!)
-Year long log to document all of the character traits learned throughout whole group/small group instruction. Having this easy to access list for students is critical! You can add to the list whole group during reading instruction and students can use this list during their narrative writing unit.
-1 section cover for each of the Common Core domains- literature, information, language, speaking and listening and writing. Students will store notes, handouts and assessments behind the corresponding tabs.
