Hold students accountable for their own response to intervention. When students struggle with objectives, consider offering this form as an assignment.
Students or teachers identify a problem, question, and/or measurable skills essential to be proficient.
Students earn up to 100 points by completing tasks of knowledge, reasoning, demonstrating plans to solve the problem identified, and developing an original product.
The document is two pages. One page has a grid included that can be beneficial for tables and graphs. The second page avoids a grid with additional space to write.
