When attempting to contact parents/guardians for an IEP meeting it is vital to record your attempts in case contact is not returned. This document can be attached to a student’s IEP and shared with other stakeholders.
In the date column, record the date the attempt was made or the letter was sent.
In the method column, check the method used to contact the parent(s)/guardian(s).
In the response column, write yes or no.
In the comments and notes column record things like the results of the contact, if a voicemail was left, the student forgot the note at school, the email was returned, the email was recorded as opened, the video conference was interrupted, the text message was returned, a person left the in-person meeting, etc.
You may want to consider printing this form onto duplicate or triplicate paper so multiple copies can be retained and added to a student's file.
