When attempting to contact the school for a meeting, it is vital to record your attempts especially in cases when contact is not returned.
In the date column, record the date the attempt was made or the letter was sent.
In the method column, check the method used to contact the school district.
In the response column, write yes or no and when the response was received. You may want indicate the cordiality of the response as well.
In the comments and notes column record things such as the results of the contact, the mood of the contact, who delivered a particular letter, if a voicemail was left, the email was returned, the email was recorded as opened but unreturned, the video conference was interrupted, the text message was returned, a person left the in-person meeting unexpectedly, etc. You may want indicate the cordiality of the response(s) as well.
In the person(s) contacted column, write the name of the person(s) whom you attempted to reach.
