As a teacher, I use this form to quickly jot down which skills, activity, or worksheet that you would like a volunteer to help a student with. Then when a volunteer comes to help, they will know exactly what to do (and not have to interrupt your lesson to ask how to help). The best part is the place for volunteers to write notes about their time with your student(s). Often I don't have time (or privacy) to debrief with volunteers about their time spent with my kiddos. This takes the guess work out of how their interaction went, and gives me documentation of time spent working with another adult on a particular skill.
