The IEP at a glance sheet includes all of the important information needed by special educators and general education classroom teachers to provide the services and accomodations/modifications required by students' IEPs. There is a general student information section, followed by a section for academic and behavioral accomodations/modifications and services, followed by a breakdown of services, service hours, days, locations and providers.
The progress monitoring sheet is a great way to record and communicate students' IEP goals and objectives and the baseline data for each (as well as indicating the assessment used and to be used for further collection, the students goal, when to assess/collect data, when assessments are actually given and the results of the assessment). I run off copies of these for each academic/behavioral area of impact for my kiddos and put them in a binder. I use them to gather data throughout the year and determine students' progress. Great all-in-one tool.
