I use this chart in fourth grade to keep track of the students' behavior in each of their classes. We have two teachers this year; therefore, there is a column for each teacher in order to know who the student got a check from that day. I also highlight the boxes of students' names based on their homeroom teacher; for instance, my homeroom students' boxes are highlighted blue and the other teacher's boxes are pink. The students are able to receive one warning a week. We place a "W" on the board so both teachers know the student already received their warning. At the end of the week, we count the checks for each student and place them in the total column. The following week, we send home each student's conduct with their paper packets. I also have attached this weekly conduct report sheet that we staple to their paper packets. I hope you find this product useful.
