When I was teaching 7th grade, we had to create a conduct chart that would work for us. Along with a friend, we felt this was the most conducive to our environment - one that targeted the areas that needed work in our classes. It can easily be adapted or changed to what your needs are as far as categories. You can copy and paste the names in and it is ready to go.
I now teach high school and though they are not concerned as much with conduct as they used to be, I always have something tangible to show where I got their grade and that I didn't just make it from thin air.
This chart is great documentation for parents conferences and office data.
