This form is to be used by any teacher outside of the homeroom classroom that wants to know what is going on in the student’s classroom. Last year I was the gifted teacher for our school district and I had kiddos on a one day a week pull out schedule. The communication between classroom teachers and intervention (whether it be gifted, math, reading...) is often low. I needed an insight into their classroom. Who better to ask than the students themselves? I had my kids fill out the form in the first 5 minutes of the day while I am getting ready to begin.
