This unit includes all the forms that you need in order to keep up with your students who are on your caseload as an IEP monitor or consult student. This is a great way to document your indirect speech services. It is necessary to have your consultation services documented and I have tried to make that process easier with these organized forms. They are quick and easy to use checklist, so that all involved (regular education teacher/resource teacher/speech therapist/student) do not feel overwhelmed with paperwork!
This kit includes:
*Consultation Speech Services/IEP Service Plan Form-I fill this form out at the beginning of the school year to inform all of my new teachers which of my students are on a consultation basis and what their accommodations/modifications/supplemental services are for the current school year. I have two versions of this form. One where you fill it out, initial it, copy it and hand the teacher a copy. The other where you initial it, the teacher initials it, you give a copy to the teacher and keep a copy in your speech folder.

★Teacher checklist for the annual review
★Generalized consultation observation form that you can give any member of the IEP team to fill out on the student to assess progress and consider continued need for monitoring
★Student self-assessment to determine who they are doing in class and how they feel about the monitor/consult status
★SLP documentation form for each reporting period
★SLP student list in order to track your students
These forms are perfect to take to the IEP meetings, where you will consider if speech therapy consultation is warranted.
