This is a template to use for keeping track of contact information and communication with parents. It includes:
-A Master Contact Sheet
-A Student Information Sheet (includes places for parent contact information, transportation method, preferred method of contact, medical and behavioral/learning concerns, learning strengths, permission to be photographed or featured on the class website, sibling, and other notes)
-A Parent/Guardian Communication Log (includes a place for the date, time, parent/guardian contacted, relationship to student, method of contact, reason for contact, and any notes/comments)
You can use this to keep track of contact information and logs for your students. Personally, I keep these in my student information binder. The Master Contact Sheet stays in the front for easy access, followed by a copy of the Student Information and Parent/Guardian Communication Log Sheets for each student. Any time I have contact with a parent or guardian, I fill out a log section. It really comes in handy for conference times!
