I used this sheet with my 4th and 5th graders to help them create a form using simple rhythms. They also perform what they write. Could be used as an assessment for form or for rhythms.
In each section (A, B, or C) they fill in the beat map with appropriate rhythms for their level. They then decide what order they want their sections to be in (creating their form). I did this sheet with partners, so they were to work together to write the rhythms and decide the order of the sections.
They performed their forms for the class.
I have also done this differently where we decided as a class what our 'beatmaps' would say. We have done actions instead of rhythms (i.e. clap, clap, stomp stomp). The students were then able to take those sections and create their own order for the performance.
