This is a one-sided document with 12 questions. The student is given the leading coefficient, the degree and the number of terms of the polynomial and then asked to create their own polynomial (written in standard form) that fits the criteria. Great enrichment activity for understanding the vocabulary of polynomials. At least one situation is not possible. Key included.
