Here is a success criteria rubric for Florida's Math Standards. This particular one is for:

MAFS.4.NF.1.1: Explain why a fraction a/b is equivalent to a fraction (n × a)/(n × b) by

using visual fraction models, with attention to how the number and size of the parts differ even though the two fractions themselves are the same size. Use this principle to

recognize and generate equivalent fractions. (Using denominators 2, 3, 4, 5, 6, 8, 10, 12, & 100)

I color code them to match the clip chart for self-assessing that is also in the student's notebooks for students to check their learning. This one is all together. I separate each level (1, 2, 3, & 4) and display on color coded chart in class
