In my 5th grade classroom, we practice "The Big 4" with independent reading, partner reading, response to text, and listening to reading. We also set goals with CRAFT-comprehension, responding to text, accuracy, fluency, and text enhancers.
These tracking sheets let me keep track of my students' goals and choices. They also require students to do the same in a simple way.
