This behavior chart can be used in any grade, but I suggest using it for mainly lower elementary. I use it in my Kindergarten class, and it works as a great motivator for good behavior from my kiddos! The chart goes home with the student each day. At the end of each day, each student receives a check mark in the "behavior" column of the chart: green for a great day, yellow for a day with several behavioral reminders, and red for having to sit out or being sent to the office. If I have to assign a red check, I will send a note home or talk to the parents during dismissal that day. I use the "comments" section to write anything from explaining a yellow check mark day to mentioning a highlight of a student's day. Parents are asked to sign their initials each day in the "initial" column so I know they are consistently aware of their child's behavior in the classroom. I've received great feedback from parents, and this helps to keep us all on the same page! I would suggest making several copies of the sheet for each student in your class to form a packet, or simply print a new sheet once one has filled up. These can then be sent home to parents or kept in the student's file.
