This behavior chart lets you break the day down for the child in 1 hour segments. The child can color the puzzle piece if they behave and stay on task. If they do not, you can write in the box. At the end of the day, the student counts up how many puzzle pieces are colored, and there is a place for a parent signature. You can set goals or offer rewards/consequences for the chart. These are good for data. You can look for patterns and see what time of day undesired behavior occurs. I use this chart for individual students when my regular classroom management plan isn't enough.
