Color-coded, happy-face system for a brief, quick report of child's daily behavior for the school week. Includes a comment section for reporting on "Positive" and "Negative" moments. The report is easy enough for the child to understand as well so that he/she can learn to be accountable for behavior.
I created this chart for my son's pre-k teacher to use for him, and he understands the chart well enough that when he earns a green super-smiley face, he gets 2 trains from the treasure box; when he earns a yellow smiley face, he gets 1 train; but if he earns an orange face, 1 train gets taken away, while a red face will get him 2 trains taken away. The chart works like a charm since it helps him to be accountable for his behavior with rewards for good behavior and consequences for negative behavior.
