This is the monthly behavior chart for October 2012 that goes home with the child every single day. This tells the parent if the child was on green light, yellow light, red light or the horrible pink slip. I simply highlight the child's stoplight color for the day. If the child gets a yellow light or something other than green, I make notes under the stoplight words, such as disruptive in class, bothered others, not following directions. I have been using this method for years and it has worked beautifully for me. This is great for children with behavior problems in your classroom. I have taken this chart to ARC meetings to show patterns in behaviors and issues that we have in class.
