I used this for students when behavior modification was needed. The student and the teacher choose a goal together, start with Goal A only. then you can move onto Goal B when the student has done well with Goal A.
The goals can range from "raising my hand" to "staying in my seat" to "speaking only when appropriate." I make 5 copies for the whole week and staple them together. I write the times in the boxes and give a smiley face when the goal is achieved...even if there is a little progress.
I would write in a sad face when the goal is not met. When the student has all smiley faces (or mostly smiley) they get a reward....computer time, help the teacher, a prize from a prize box etc. This needs to be sent home to the parents. I usually clear the use of this report system with the parents before I use it to help modify behavior.
